Most managers don't know what kind of pivot they have until something fails. There are three patterns we work on every week:
Offset pivots (Dorma 7400, Rixson 28, Rixson L28). The pivot pin sits offset from the door edge, usually 3/4". You see these on heavy commercial entry doors at banks, hotels, and higher-end retail along US-1 and St Lucie West Boulevard. Top pivot is mounted to the header soffit; bottom pivot drops into a floor plate. Failure modes: top pivot screws walking loose from header pull-out, bottom pivot bushing wearing oval (door drops 1/4" to 1/2"), or the offset bracket cracking on doors that have been kicked or wind-slammed.
Center-hung pivots (Rixson 117, Dorma 8000-series). Pivot is on the centerline of the door. Common on restaurant entry doors, glass storefronts, and some hotel lobbies (Hilton Garden Inn PSL, Hampton Inn). When these wear, the door wants to drift to one side and the threshold sweep starts dragging on the floor. We see this constantly on restaurants where the entry door cycles 800+ times a day.
Concealed pivot-set / floor-closer pivot. The pivot IS part of the floor closer (Rixson 27, Rixson 28, Rixson 1500, Dorma BTS). Replacing the pivot means servicing the floor box. We document this on the concealed door closer page because the pricing is much higher.

Manufacturer cycle ratings assume room-temperature, low-humidity, normal occupancy. PSL accounts get none of those:
Heat. July rooftop temps along the US-1 corridor and Tradition Square hit 130 degF on dark-bronze aluminum frames. Pivot lubricants thin out, bushings carry more direct load, and the bronze-on-steel interface wears faster. We see 2-3x the wear of a Midwest install on the same door.
Salt air. Restaurants and hotels near Hutchinson Island, the bridges over the Indian River, and the older Treasure Coast plazas at Jensen Beach pick up airborne salt that pits unprotected steel pivot shafts. Doors that should run 10 years run 4-6 before the shaft is pitted enough to bind. We use stainless replacement shafts and bronze bushings on coastal accounts.
Hurricane code doors. Impact-rated entry doors at hotels and the larger plazas are heavier (sometimes 180-240 lbs). Hardware sized for a 110-lb door fails fast under a 200-lb load. We size up the pivot rating (Rixson 117 3/4 instead of 1/2, Dorma 7400 high-load instead of standard) when the door weight warrants it.
Foot traffic at schools and grocery anchors. Treasure Coast HS, Centennial HS, the Publix at Tradition Centre Drive, the Walmart on Federal - any door that cycles over 1,200 times a day chews through standard hardware. The fix is a bushing or full bottom pivot swap, often 90 minutes on-site. The other 10 percent: top pivot screws walked loose from the header (we re-anchor into solid framing, sometimes with through-bolts to the steel beam), or the door itself has sagged at the welds, which is a glazier / commercial door company job, not a locksmith one. Both. If the shaft is straight and the housing isn't cracked we rebuild with a new bushing kit ($340-680). If the shaft is bent or pitted, or the housing is cracked at the offset arm, full replacement ($480-980 for the pair). The diagnostic is a 15-minute pull-and-inspect. We're honest about it - rebuilding a cracked housing is a callback in 6 months, so we don't do it.

PSL conditions. Dorma's 10-year cycle rating assumes climate-controlled interior installs. On a south-facing US-1 storefront the bronze aluminum frame can hit 130 degF in July, which thins the pivot lubricant and increases wear on the bushing. If the door is also impact-rated (180-240 lbs versus the 110-lb design weight Dorma rates against), you're at 2-3x the design load. We size up the pivot rating on replacement so the new one lasts longer.
Pivot hardware itself is locksmith / commercial hardware specialist territory - we own that. If the door frame is bent, the header is sagging, the welds at the door corners have cracked, or the glass is damaged, that becomes a glazier or commercial door company job and we'll refer out. About 85 percent of PSL pivot calls are hardware-only and we close them out same visit.
